AMAÇ: Hirsutizm şikâyeti ile başvuran hastalara uygulanacak kombine tedavi protokollerinin karşılaştırılması amaçlanan çalışmamızda oral kontraseptif tedaviye spironolakton veya siproteron asetat ilave edilmesinin hormon seviyeleri ve klinik tablo üzerine etkinliklerini karşılaştırmak.MATERYEL ve METOD: Trakya Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Polikliniği'ne hirsutizm şikâyetiyle başvuran 69 hasta çalışmaya alındı. İki gruba ayrılan hastalarda bir grubuna (Grup 1, n=34) kombine siproteron asetat 2 mg ve etinil östradiol 35 µg içeren oral kontraseptif ve spironolakton 100 mg/gün, diğerine ise, (Grup 2, n=35) aynı kontraseptife siklusun 1.-10. günü siproteron asetat 50 mg/gün ilave kombinasyonu uygulandı. Tedavinin başlangıç, 6. ay ve 1. yılında her iki tedavi rejiminin hirsutizm skorları ve hormon düzeyleri karşılaştırıldı.BULGULAR: Her iki tedavi protokolünde de hirsutizm skorunun azalmış olduğu ve bu anlamda klinik düzeydeki iyileşmenin her iki tedavi grubunda da aynı oranda olduğu görülmüştür. Tedaviler sonrası serum serbest testesteron ve DHEAS değerleri, her iki grupta da anlamlı olarak azalırken, SHBG düzeylerinin anlamlı olarak arttığı görülmüştür. Total testesteron sadece sipronolakton ilave edilen grupta anlamlı azalırken, serum androstenedion düzeylerinde ise, her iki tedavi sonrasında da anlamlı değişim olmamıştır.SONUÇ: Her iki ilaç kombinasyonu ile de benzer klinik ve anti-androjenik etkinlik sağlanmaktadır. İlaç seçiminin, hastaların yan etki spektrumuna göre yapılması önerilebilir.
OBJECTIVE: To compare clinical effectiveness and effects on hormonal levels of the combined drug therapies of oral contraceptives plus spironolactone or cyproterone acetate on patients have complaints with hirsutism.STUDY DESIGN: 96 Hirsute women referred to Trakya University Medical Faculty, Department of Obstetrics&Gynecology were included into this study. Of the 69 patients, 34 of them (Group 1) treated with combined oral contraceptive including Cyproterone acetate 2 mg ve etinyl estradiol 35 µg and spironolactone 100 mg/gün, 35 of them treated with the same contraseptive combined with high dose cyproterone acetate (50 mg/gün). Clinical hirsutism skores and serum androgen levels were evaluated at the beginning, sixth months and the end of the first year.RESULTS: We showed that both of the treatment modalities decreased hirsutizm scores and improved clinical complaints in the same effectiveness. At the end of the treatment, serum free testesterone and DHEAS levels decreased , SHBG levels increased significantly in the both treatment groups. While serum total testesterone levels decreased only in OC plus spironolactone treated group significantly, there were no significant difference in androstenedione levels after the both combined therapies.CONCLUSION: Both of the drug combinations were the same clinic and anti-androgenic effectiveness, So that, we could prefer one of the therapies in light of the side effect spectrum.
