The present case report describes an elderly patient with complete atrioventricular nodal block coexisting with intermittent preexcitation, which resulted in paroxysmal complete loss of atrioventricular conduction and symptoms. The patient ended up with pacemaker implantation without requiring ablation of accessory pathway. © 2008 Italian Federation of Cardiology.
