Özet
Objective: To evaluate whether administration of additional dose of metronidazole vaginal ovules at the vaginal apex reduces the risk of postoperative febril morbidity and vaginal cuff cellülitis after abdominal total hysterectomy on benign indications. Material and Methods: This prospective placebo-controlled clinical study was carried out at the Department of Obstetrics and Gynaecology of the Bakirkoy Women's and Children's Teaching Hospital Istanbul, Turkey from January 2009 to January 2011. The study sample consisted of women who had abdominal total hysterectomy on benign indications. Participants were assigned two groups. In the metronidazol group, 306 women administered additional surgical preparation of metronidazol vaginal ovules at the vaginal apex as two doses before 12 and 1 hours of the operation. In the control goup, 305 women underwent standart surgical procedure. The primary outcome measure was the rate of febril morbidity after abdominal hysterectomy. The secondary outcome measure was the rates of vaginal cuff selülitis and pelvic abscess. Results: Participants in both groups had similar demographic and clinical characteristics. The rates of febril morbidity(16.7% vs 19.0%, p=0.448), pelvic cellülitis (3.6% vs 6.2%, p=0.132), pelvic abscess (0.3% vs 0.7%, p=0.561) and wound infections (7.5% vs 8.2% p=0.755) after abdominal hysterectomy were lower in the metronidazol group but this difference was not significant. Furthermore the rate of pelvic hematoma was equal in both groups (0.3%). Conclusion: Vaginal preparation by using metronidazol vaginal ovules before the operation did not effective to reduce the risk of postoperative febril morbidity and pelvic cellulitis.
