Özet
Purpose: To assess radiation - induced changes in lung function of patients with breast cancer receiving radiotherapy (RT). Patients and methods: Twenty patients with breast carcinoma were included in this prospective study. Lung perfusion scans (PS) were used to assess regional changes in lung perfusion, and pulmonary function tests (PFTs) were used to assess whole-organ function changes. PFTs included forced expiratory volume at one second (FEV1), vital capacity (VC), forced vital capacity (FVC), and maximum mid-expiratory flow rate (FEF25-75). PS and PFTs were obtained before RT and at 3-month intervals and compared with pretreatment values. Results: Lung metastases occurred in 2 patients, who were excluded from the study. Respiratory symptoms secondary to RT occurred in 2 (11%) of 18 patients. There was no significant change in PFTs between pre- and post- treatment values. Reduction in apical lung perfusion was seen in 16 (89%) of 18 patients at 3 months post-RT and in 15 (83%) of 18 patients later than 6 months post-RT. Apical region lung perfusion in the irradiated side decreased until 3 months post-RT and partly improved by 6 months. It did not, however, reach pre-treatment levels. Conclusion: Radiation-induced pulmonary symptoms are not common in breast cancer patients receiving RT. There is a significant functional reduction in the apical region of the irradiated side, but this reduction is clinically well tolerated.
