Özet

Amaç: Çocuk istismarı ve ihmali yaralanma, kalıcı hasar hatta çocuk ölümleri gibi ciddi sonuçlarının yanı sıra çocuklarda psikososyal hasar da bırakan, tıbbi, hukuki ve toplumsal yönleri olan ciddi bir halk sağlığı sorunudur. Bu çalışmanın amacı, Trakya Üniversitesi Tıp Fakültesi altıncı sınıf öğrencilerinin çocuk istismarı ve ihmaline yönelik farkındalık düzeylerinin ve bunu etkileyen faktörlerin incelenmesidir. Gereç ve Yöntemler: Kesitsel ve tanımlayıcı tipteki bu araştırmaya tıp fakültesi son sınıf öğrencisi olan 184 kişi dahil edilmiştir. Katılımcılara sosyodemografik özelliklere, çocuk istismarı ile ilgili eğitim ve mesleki tecrübelere ilişkin soruların yer aldığı “Anket Formu” ile “Çocuk İstismarı ve İhmalinin Belirti ve Risklerinin Tanılanmasına Yönelik Ölçek Formu” uygulanmıştır. Bulgular: Araştırmaya katılan 184 öğrencinin % 51,6’sı erkek ve %1,6’sı tek ebeveynli ailede büyümüştür. Çalışmanın sonuçlarına göre katılımcıların % 85,9’u tıp eğitimi süresince çocuk istismar ve ihmali konusunda eğitim aldığını belirtirken; eğitim alanların % 58’i aldığı eğitimi yetersiz olarak değerlendirmektedir. Katılımcıların Çocuk İstismarı ve İhmalinin Belirti ve Riskleri Tanımlama Ölçeği’nden aldıkları ortalama puan 4,02±0,02’dir. Buna göre katılımcılardan tıp eğitimi süresince eğitim alanların ölçekten aldıkları puan ortalamaları almayanlara göre yüksek saptanmıştır (p=0,007). Katılımcıların istismar/ihmal vakası ile karşılaşma durumları ile ölçekten aldıkları ortalama puanlar karşılaştırıldığında istismar/ihmal vakası ile karşılaşan kişilerin ortalama puanları karşılaşmayanlara göre yüksek bulunmuştur (p=0,008). İstismar ve ihmal ayrımı yapabilme sorusuna “hayır” diyenlerin ölçekten aldıkları ortalama puanlar, “kararsızım” ve “evet” diyenlerin ortalama puanlarından anlamlı olarak düşük saptanmıştır (sırasıyla p değerleri p=0,012; p=0,018). Sonuç: Katılımcılardan tıp eğitimi süresince çocuk istismarı ve ihmaline yönelik eğitimi alanların, istismar/ihmal vakası ile karşılaşanlar, istismar/ihmal ayrımı yapabilenler ve 183 çağrı numarasını bilenler Çocuk İstismarı ve İhmalinin Belirti ve Riskleri Tanımlama Ölçeği’ nden daha yüksek puanlar almışlardır.
Objective: Child abuse and neglect is a serious public health problem with medical, legal, and social aspects, leaving psychoso- cial damage to children as well as serious consequences such as injury, permanent damage and even child deaths. The aim of this study is to examine the level of awareness of Trakya University Medical Faculty sixth year students about child abuse and neglect and the factors affecting it. Material and Methods: This cross-sectional and descriptive study included 184 intern medical doctors. The “Survey Form”, which in- cludes questions about sociodemographic characteristics, education, and professional experience related to child abuse, and “the Diagnosis Scale of the Risks and Symp- toms of Child Abuse and Neglect” were applied to the participants. Results: 51.6% of the 184 students participating in the study were male and 1.6% grew up in a single-parent family. Accord- ing to the results of the study, 85.9% of the participants stated that they received training on child abuse and ne- glect during their medical education, while 58% of the trainees evaluated the training they received as insuffi- cient. The mean score of the participants in The Diagno- sis Scale of the Risks and Symptoms of Child Abuse and Neglect was 4.02±0.02. Accordingly, the mean scores of the participants who received training during their medi- cal education from the scale were found to be higher than those who did not (p=0,007). When the participants’ en- counters with abuse/neglect cases and the average scores they got from the scale were compared, the average scores of those who faced abuse/neglect were found to be higher than those who did not (p=0,008). The average scores of those who said “no” to the question of being able to dis- tinguish between abuse and neglect from the scale were found to be significantly lower than the average scores of those who said “I am undecided” and “yes” (p values are p=0.012, p=0.018, respectively.) Conclusion: During the medical education of the participants; those who received training on child abuse and neglect, encoun- tered a case of abuse/neglect, could distinguish between abuse and neglect, and/or knew the 183 call number re- ceived higher scores from the Child Abuse and Neglect Scale for Identification of Symptoms and Risks.
