Lung cancer commonly metastasizes to bones, primarily the axial skeleton, but rarely to the foot, specifically the calcaneus. We report a case of a 63-year-old male with heel pain and swelling, previously treated for lung cancer. Isolated acral metastasis of lung cancer has an incidence of 0.1-0.7%, predominantly affecting the hindfoot. Diagnosis involves clinical evaluation, imaging, and biopsy. Treatment options include surgery, radiation, and chemotherapy. Calcaneal metastasis, though rare, should be considered in lung cancer patients with heel pain for timely management.
